This is regarding my son, aged 10 years who is having Hypospadias since birth. He was operated at the age of 3 at a "leading" Hospital in Chennai. However after 3 months of operation the defect has returned. The Operating surgeon to whom we are going for yearly check-up says he will require another surgery. But that will be done only at the age of 15... when the organ grows a bit bigger. WE ARE VERY WORRIED. I have a feeling that his Organ is too small for his age. Recently he has put-on some weight. His height is 125 cms and weight is 45 Kgs. He is generally Healthy and good in studies. I want to take a second opinion... Kindly advise

Plastic surgery can surely help people with ugly/noticeable scars by "revising" or doing "Scar Revision". In most instances scar cannot be fully removed. But by utilizing various methods/techniques it is possible in most instances to make the scar less obvious or rarely even "almost" invisible. The final result depends on so many variable in the patient and also to some extent expertise and experience/skill of the Plastic Surgeon too. I suggest that U directly consult your Plastic Surgeon and have a free and frank discussion with him as to what are the options and the possible outcome. U R welcome to share the feedback after you consult your Surgeon.

Once a stone former, it may recur and hence U have to take precautions. Most important is to take plenty of water, which U R doing. Do let me know what is u'r height and body weight. If someone is not putting on weight at all, U have to consult u'r doctor to make sure there is no underlying disease responsible for it - like Diabetes, Thyroid hyper function or tuberculosis. So have a medical check up. If all these are ruled out all U have to do is to lead an active life - plenty of physical exercise (yes, this is also important, to build up muscles and appetite) and more nutritious food. A good plan is to eat some snacks like Cream biscuits, other savouries, nuts like cashew, ground nuts, almond, walnut etc and also sweets, ice cream etc about 1/2 to 1 hour after a regular meal, say while watching TV etc- keep munching. Also take more hot drinks like Coffee, tea, Horlicks, Bourn Vita etc with more sugar. Avoid eating anything for 60-90 minutes before u'r main meals so that U do eat a good meal. Also make sure U get a balanced diet with plenty of fresh vegetables as salad and fresh fruits 1 hour before each meal. BUT DO BEAR in mind, NOT TO OVERSHOOT and become too fat/over weight. AND NOT TO WORRY AT ALL even if U don't put on much weight.. JUST LEAD AN ACTIVE LIFE. Better to be thin than over weight.

1.5 years after marriage is too short a period for u to worry at all, if U do not have any other medical problem. However,yes, surely U can get yourself evaluated reg your fertility. Stress is part of life for everyone. It is just worrying about one's day to day domestic or professional problems and all that unnecessarily. What U need to do is to take a philosophic view of life and face whatever real or imaginary problems u face and tackle it to the best of u'r ability. Just think - what is the worst that is going to happen if a problem is not/could not be solved? And also just think for a while and decide how U can tackle the problem? U may even write down how U R going to tackle the problem next day - before U retire to bed at night. Next day just do what U have decided and good/reasonably good results under the circumstances will follow. Yes, YOGA - meditation, relaxation etc will surely help. U can't say U have no time. It is like not having time to fill petrol for U'r bike because u R too busy travelling!! U have to recharge u'r spirits and morale and Yoga will surely help. U have to take time out for it. The main cause for fissure is hard motion - harder than what one is accustomed to and hence results in excessive straining. Fissure present for months would be called "Chronic" and associated with lot of scarring and quite often(but not always) needs surgery. It is best for U t directly consult a Surgeon. Liver cancer may be primary in Liver (Starting in liver itself) or Secondary (Spread to Liver from some where else). Either way it is to be treated in a Speciality Hospital dedicated to Cancer. It is better that U take the patient to a larger Institution for Cancer like cancer Institute, Chennai, Tata Memorial in Mumbai or Gujarath Cancer Institute in Ahmedbad etc.

I'm delighted to be of service to U. I'm sending U the Standard reply that we send for this very common enquiry. U may read all details about Gynaecomastia and surgery for it in our site as Newsletter on page Newsletter under Archives. We do liposuction under full general anaesthesia for this and U have to be in Hospital for 2-3 days. Surgery gives U permanant releif; the condition does not recur. There will be a minute scar fairly well hidden, so that it is difficult to find out, after some time, that U got operated. Risks: there is usually no special risk - it is similar, what is called as "standard risk", to any other surgery of this magnitude. Surgery for it is generally Not covered by insurance.

The total costs ( IN OUR HOSPITAL) could be in the range of Rs 25 to 50 thousands on average (and could be more in other Hospitals), according to the Surgeon and the Hospital. U may confirm exact figure at the time of I consultation with your surgeon. If U R doing sedentary/desk work, U could go back to work in 3-4 days' time or surely in a week's time.

If U R not keen to get it operated, it may be left alone & it will not have any adverse effect on your health. There may or often may not be associated hormone imbalance and this will be untouched/unaffected even after surgery. This aspect may need attention/treatment by an endocrinologist. But even if U take this treatment before surgery, it will not reduce size of breasts. Your surgeon will be able to guide U on this matter.

You have sought treatment for Fistula in ano (there is no grading for it) and whether it can be performed in our Hospital.

Fistula in ano has to be treated by surgery only; if it is relatively low or mid level it can be cured in one sitting. In case it is a "high" fistula (or anorectal fistula) you may have to undergo 2 stages. For this surgery you have to fix appointment with our Chief in Surgery, Dr Ramesh Kumar Mohan Rao. Not knowing full details of surgery done and patient's overall condition, it is difficult to advise on the net.
However, his pain/irritation/burning while passing urine suggests still some infection in urinary tract.
Other symptoms like cough, shortness of breath etc may be due to and Pulmonary(Lungs) or Heart problem.
How old is he? What was the original problem and what surgery was performed and when? Is he a Diabetic, Hypertensive (BP), Asthmatic??
He should directly consult a Urologist as well as a Consultant in Internal Medicine.

Yes he is diabetic, his age around 56 , he has hypertension and wheezing cough after the surgery of urine obstruction fat removal. After getting Diabetes under control, Circumcision surgery can be performed as daycare surgery (no need to admit in Hospital). The total cost for this surgery IN OUR HOSPITAL would be about Rs 15 thousands. But sometimes in a Diabetic we may have to admit for a day or two and for this there will be some more charges for in-patient stay in Hospital. Otherwise costs vary considerably from Surgeon to Surgeon and Hospital to Hospital.

In most instances it is possible to make complete corrective surgery satisfactorily by 4-5-6 yrs or so.
But in case the size of penis is relatively less (acc to U), the Surgeon will have to see and tell U if it is really small and under developed and if so it may be due to some hormone imbalance . If this is so hormones need to be supplemented early in life, but under careful supervision and regular monitoring by a Qualified and experienced Endocrinologist. The hormones supplementation may not be possible if U wait too long till 15 years!
Once this hurdle is overcome then the surgery should be possible preferably someone with lots of experience in THIS PARTICULAR SURGERY. For this I suggest Dr R Sanjay Rao ( our Hospital's former Paediatric Surgery Consultant) currently in Narayana Hridayalaya in Bangalore. If U want to consult him, please give me full details of previous surgery (if available) and also the present position including some photographs of the affected area.
